About the Opportunity

A nationally recognized academic medical center is seeking an Outpatient Oncology Pharmacist to join its cancer care team. The pharmacy department includes more than 500 staff members dedicated to providing both inpatient and outpatient pharmacy services, ensuring patients receive high-quality care tailored to their needs.

The organization’s cancer enterprise is at the forefront of oncology care, consisting of two cancer centers and three affiliate sites. Oncology pharmacists collaborate with expert cancer physicians, researchers, and nurses to advance leading-edge research and treatment innovations, making a significant impact on patient outcomes and the future of oncology treatment.

As an Outpatient Pharmacist, you will be an integral part of the patient care team, working closely with medical, nursing, and pharmacy staff to provide exceptional care to cancer patients.

Key Responsibilities

• Patient Care: Provide comprehensive care for outpatient oncology patients across cancer-related disease states, with a focus on hematologic malignancies, bone marrow transplant (BMT), CAR-T, and TIL therapies
• Order Review and Verification: Review and verify chemotherapy orders to ensure accuracy and safety
• Patient Education: Educate patients on their treatments and care plans
• Therapeutic Drug Monitoring: Monitor therapeutic drugs to optimize therapy and improve outcomes
• Guideline Development: Participate in oncology disease-related groups supporting guideline and SOP development, updates, and new drug monographs
• Drug Information: Provide accurate drug information to healthcare professionals and patients

Qualifications

Education

• D. or equivalent degree from an accredited school of pharmacy
• Completion of a PGY1 and/or PGY2 residency, or equivalent practice experience

Licensure and Certification

• Current pharmacist licensure in the State of California
• BLS certification

Preferred Experience and Skills

• Previous clinical oncology or infusion pharmacy experience
• Board Certification in Oncology (BCOP)
• EPIC/Beacon experience
• Comprehensive knowledge of common disease states and pathologies
• Ability to extract pertinent information from the patient chart and relate it to the care plan
• Ability to apply current pharmacological and biopharmaceutical principles to drug selection and use in a clinical setting
• Ability to evaluate medication regimens against patients’ therapeutic needs
• Ability to learn and adapt to automated information systems in daily practice
• Exceptional communication, interpersonal, organizational, prioritization, and follow-through skills
